Nottinghamshire Mind’s Resilience and Stabilisation Programme is available on referral from practitioners within the county Local Mental Health Teams (LMHT) and Community Mental Health Teams (CMHT) at Nottinghamshire Healthcare NHS Foundation Trust.
This referral form is ONLY for the use of these practitioners. Referrals submitted by any other service/practitioner will not be accepted and will be rejected.
